Privacy Policy

This Privacy Policy explains what information Final Form Research LLC (“Final Form,” “we,” “us”) collects through finalform.bio (the “Site”), how we use it, and who we share it with. It does not apply to any other site or service.

Information We Collect

When you create an account, we collect your email address and password. When you place an order, we collect the billing and shipping information you provide (name, address, phone, email). If you register as a reseller, we also collect your referral code and any payment methods you choose to display to buyers referred by your link. We do not collect or store payment card numbers, since we don’t currently process card payments; any payment you send is made directly to us through the method you choose at checkout.

Cookies

We use a small number of functional cookies, not advertising or tracking cookies:

• An age/researcher verification cookie, set once you confirm the entry attestation, so you aren’t asked again on every visit.
• A reseller referral cookie, set for 30 days when you arrive through a reseller’s link, so that reseller’s pricing applies to your visit.
• Standard WooCommerce cart and session cookies, needed to keep items in your cart and process checkout.

How We Use Your Information

We use the information we collect to create and verify your account, process and fulfill orders, send order and account-related email, attribute orders to the correct reseller, and maintain compliance records, including the timestamp, IP address, and text of the liability release you agree to at registration, kept as a record that you agreed to it.

Who We Share Information With

We do not sell your information. We share it only with service providers who need it to run the Site, currently:

• SendGrid, which sends transactional email on our behalf (order confirmations, account verification, password resets).
• Our web hosting provider, which stores the Site’s database and files.

We may also disclose information if required by law, or to investigate suspected violations of our Terms of Service.

Data Retention

We retain account and order information for as long as your account is active and as needed to maintain compliance and business records. You can request deletion of your account by contacting us, though we may retain order records as required for legal, tax, or compliance purposes.

Your Rights

You can request a copy of the information we hold about you, ask us to correct it, or ask us to delete your account, by contacting support@finalform.bio.

Age Requirement

This Site is not directed at, and we do not knowingly collect information from, anyone under 21. Entry to the Site requires confirming you meet this requirement.

Changes to This Policy

We may update this Privacy Policy from time to time. Changes will be posted on this page.
